- A 23-year-old woman,at 38+2 weeks gestation, presented with complaints of headache, giddiness, and vomiting for 2 days.
Ultrasound suggested a single intrauterine fetus in cephalic presentation with a gestational age of 39 weeks and 2 days.
- CT brain and angiography revealed a ruptured basilar apex aneurysm.
The patient underwent a left FTOZ craniotomy and clipping of the aneurysm through a half-and-half approach.
The basilar top aneurysm was successfully clipped in 45 minutes.
- Due to fetal bradycardia, an immediate caesarean section was performed by the OBGYN team, and the baby was delivered.
- On post-operative day 2, mother was extubated. She experienced transient hemiparesis, which resolved by the 3-month follow-up. Both mother and baby are doing well at follow-up.
- A 64 year old female presented with ,altered sensorium and severe headache . Hunt and Hess grade 3, with SAH Fischer grade 4 . CT angiography was performed and showed right ruptured superior hypophseal and left mca aneurysm.Both aneurysms were tackled in single setting. Patient did well postoperatively and was discharged.
